Dear Ms. Sureva,
Perhaps, some of the US Government releases you mentioned were dated before October 2006. Here is why:
On October 3, 2006, the Board on Geographic Names (BGN) unanimously voted to change the BGN standard transliteration of the name of the Ukrainian capital to ‘
Kyiv’. This decision affected the whole US Government. Although ‘Kiev’ remained the BGN conventional name for this city, all USG offices and operations were requested to immediately begin using the new spelling ‘Kyiv’ in all written communication. The American diplomatic post there was to be called
Embassy Kyiv.

I assume that the US airline ticket you mentioned was issued by a
private enterprise, which uses the conventional name of the city. Whereas in US Government official documents the official name with the new spelling is to be used.
I greatly appreciate your sending me hyper-links or any other references to the specific USG releases with incorrect spellings you found. If you wish, you may reply directly to me without posting to the list. Thank you very much.
