ZdravPlus

Project Focus:  Health Care Policy and Reform, Health Management, Primary
Care, Reproductive Health, Health Care Finance, Evidence Based Practice,
Infectious Diseases, Information Systems

Abt Associates has worked in Central Asia since 1995 under three
consecutive projects – ZdravReform (1995-2000), ZdravPlus (2000-2005),
and ZdravPlus II (2005-2009).  These regional projects, funded by USAID,
have helped the governments of Central Asia improve the financial
sustainability, efficiency, and quality of their health-care systems while
preserving equitable access.  The current project continues to provide
technical assistance to increase the efficiency of resource use, improve
health-care quality, redefine patient rights and responsibilities, and
create a favorable legal and policy framework for health reform.  In
Kyrgyzstan, the introduction of a new health financing system created
system-wide efficiencies, increasing funding for outpatient care and
medicines and reducing the excessive number of buildings and staff.  The
new system increased cross-subsidies and risk protection for poorer
populations, resulting in better access to care for the poorest
populations, and substantially reducing the negative impact of key
illnesses.

Armenia Social Transition Program (ASTP)

The Armenia Social Transition Program (ASTP), funded by USAID and
implemented by PADCO and Abt Associates, has provided support and
technical assistance to the Government of Armenia for social and health
sector reforms since 2000.  ASTP is helping the Government to develop an
integrated legal, regulatory, and information framework that supports
sustainable social insurance programs, provides needy people with adequate
social assistance, and helps improve primary health care for all
Armenians.

Central Asia Sport and Health Education Program

The Sport and Health Education Program aims to mitigate sources of
potential conflict in the Ferghana Valley and encourage interaction among
the area’s youth through cross-border community-based Game Days.  The

Toolkits for Strengthening Primary Health Care

In Albania, the PHRplus Project developed and tested a series of tools
designed to introduce family medicine concepts and strengthen primary
health care (PHC) services. Toolkits were developed and tested in four
pilot PHC centers in one region, and are now ready to be used in
additional PHC settings in Albania or adapted for use elsewhere. PHC
facility managers will find the toolkits useful reference materials when
developing strategies and tools to improve quality of care and monitor and
evaluate PHC strengthening efforts. This series comprises three toolkits:
(1) PHC Service Delivery Toolkit; (2) PHC Quality Improvement (QI)
Toolkit; and (3) PHC Health Information Systems (HIS) Toolkit. 158 pp.

Descriptive Background to Health Care Financing Reform Strategy
Development in Georgia

The purpose of the paper is to contribute to the current thinking within
MoLHSA and the Government of Georgia (GoG) about health sector financing
reforms. The paper takes a historical perspective in order to set the
stage for a common understanding of the issues inherited by the health
financing system of Georgia from the pre-“rose revolution” period. It
aims to bring these issues into the policy discussion process, which is
currently being set by the MoLHSA, and which involves all stakeholders
(national experts and policy makers as well as international development
partners). The paper attempts to identify clear priorities for the focus
of health financing policy during coming years, and to frame the debate in
order to carefully analyze the policy options available to the GoG for
developing a realistic and appropriate strategy for health care financing.

Organization and Financing of Primary Health Care in Albania: Problems,
Issues, and Alternative Approaches

The USAID-funded PHRplus Project is developing model PHC clinics in four
sites in one region of Albania in an effort to demonstrate ways to improve
systems performance. One element of the project is to assist the
Government of Albania to design and implement improved methods for the
planning, budgeting, and financing of these PHC services. A major part of
the technical assistance in these areas has been to analyze the complex
and disparate sets of data on flows of funding, how they have changed, and
how those changes have affected accountability for PHC systems
performance. Using these data, this report develops the basis for
designing alternative ways to organize and manage the PHC service delivery
system, taking account of a recent change in government policy that would
unify all funding for PHC in one agency—the Health Insurance Institute
(HII). After developing and applying criteria for choosing among two
options for reorganizing management of PHC, assuming the HII would become
the single source of financing, this paper recommends an alternative that
would create regional health offices to supervise and manage PHC in each
region, with increased levels of autonomy for individual PHC practices.
The report also describes the principal elements of a proposal for a pilot
project to test in one region the implementation of the recommended
reorganization of PHC using the HII as a single source of PHC financing.

Primary Health Care Reform in Albania: A Pilot Project to Provide Evidence
for Health Policy

The U.S. Agency for International Development-funded Partners for Health
Reformplus Project provided technical assistance to the government of
Albania from 2001 to 2005 in the design and implementation of a sub-set of
its health sector reform strategy, focused on strengthening the primary
health care (PHC) sector. An integrated PHC service delivery model was
designed, implemented and evaluated in two districts to inform national
health policy, for later refinement and replication on a wider scale. The
model integrated five major components - family medicine training for PHC
providers, a facility-based health information system, service delivery
reorganization and quality improvement, financing reforms, and community
involvement. The model addressed the low quality of primary health care,
lack of any data on PHC patients or costs, bypassing of PHC clinics for
specialty polyclinics or hospitals, fragmented financing and management of
PHC, and the absence of community involvement. The pilot was a bottom-up
approach that informed the development of an integrated PHC delivery
system that is affordable to the Albanian economy. 78 pp.

The Pilot Process: Case Study on Piloting Complex Health Reforms in
Kyrgyzstan

The concept of piloting has been effectively used in implementing health
sector reform throughout the former Soviet Union. One of the first pilots
in Central Asia, established in 1994 in Issyk-Kul oblast (province or
state) in Kyrgyzstan, continues to provide valuable information on the
process of piloting complex health reforms. The primary objective of the
pilot in Issyk-Kul oblast was to develop or refine a health system design,
specifically to demonstrate the feasibility of a mandatory health
insurance scheme. Other objectives included demonstrating specific reform
designs to provide information and evidence to national stakeholders over
time and to simultaneously build capacity for further implementation. This
case study describes the pilot process in Kyrgyzstan, including factors
involved in the development of the pilot approach, as well as dimensions
and outcomes of the pilot. 49 pp.

An Overview of GEOVAC: A Software Application to Monitor Immunization
Performance in Georgia

The GEOVAC software application is a tool designed to help personnel of
regional level centers of public health and the National Center for
Disease Control in Georgia process a large flow of immunization-related
data in much less time than the previous (manual) system. It allows them
to quickly identify issues and deficiencies regarding immunization
coverage, and use and distribution of vaccines, and to assess adequacy of
supplies as well as major barriers (medical contraindications, parental
refusals, etc.) to the functioning of the immunization system. In doing
so, GEOVAC gives health workers more time to focus on the utilization of
MIS data for management and disease outbreak response purposes. This
second version of the application has gone through numerous revisions and
suggestions based on testing in the pilot region. It is now being used
nationwide. This document illustrates GEOVAC functions, relating them to
the features of the upgraded Georgian immunization information system and
demonstrating what it can offer immunization managers in the decision
making process. It is designed primarily for policymakers in countries
planning to strengthen their immunization and/or surveillance systems,
donor organizations that can support such reforms and agencies working in
these technical areas. It can also help policymakers and health workers in
Georgia to plan and implement similar reforms in other sectors of the
health care system. 35 pp.

Assessment of Private Sector Prospects for Reproductive Health and Family
Planning Products and Services in Ukraine

The USAID/Ukraine mission requested technical assistance from the private
sector program core task order in assessing the ability of the private
sector to ensure sustained contraceptive supply in Ukraine. In particular,
the PSP-One team was expected to address the current and estimated future
availability, affordability, and accessibility of contraceptives in the
private sector. This report presents the team’s findings, provides
recommendations for activities and policies that can increase private
sector engagement in the contraceptive field, identifies specific
population groups not likely to be adequately served by the private
sector, and suggests strategies to meet their needs while avoiding
negative impact on the growing contraceptive market. 33 pp.

strategy with youth. Topics include conducting a needs assessment,
building youth/adult partnerships, and mobilizing the community. Case
studies of successful activities and strategies within Eastern Europe and
Central Asia are included. 56 pp.

Protecting the Human Rights of Injecting Drug Users: The Impact of HIV and
AIDS

This International Harm Reduction Development Program (IHRD)-produced
volume argues that unduly strict interpretation of UN drug control
treaties undermines HIV prevention efforts by discouraging countries from
implementing effective, realistic, and compassionate public health
policies. The publication's contributors—including Open Society
Institute president—demonstrate that international bodies and national
governments must give greater consideration to the human rights -
particularly access to HIV treatment - of injection drug users. By
incorporating a human rights perspective into the process of reforming the
laws and treaties governing drugs, the UN and national governments could
greatly improve access to treatment and better protect the health of those
at risk for HIV transmission, especially injection drug users. 32 pp.

ToolKit for Targeted HIV/AIDS Prevention and Care in Sex Work Settings

The purpose of Toolkit for targeted HIV/AIDS prevention and care in sex
work settings is to make both published and unpublished of this
information more accessible to a wider audience, and so to contribute to
global efforts to develop and scale up effective HIV interventions in sex
work settings. Most of the items in this toolkit focus on HIV prevention
in such settings. Less information is available on treatment, care and
support for sex workers living with HIV. Tools relating to these matters

New Harm Reduction Policy (green paper)

International Center for Policy Studies and the All Ukrainian Harm
Reduction Association have prepared a Green Paper to be discussed by
stakeholders. The Paper analyzes the roots of current problems and
proposes three policy options in response to this looming threat:
maintaining current policy towards IDUs; strengthening persecution of

Nurses’ Attitudes towards Computers: Cross Sectional Questionnaire Study

The study was conducted at two Croatian hospitals where hospital
information systems have been implemented. The results emphasize the need

Promoting Social Inclusion and Tackling Health Inequalities in Europe. An
overview of Good Practices from the Health Field

This publication presents an overview of evidence gathered from 52 good
practices that reveal how the health sector can foster social inclusion.
It reflects the outcome of a project involving twelve public health
agencies and institutes across Europe, as well as the European Health
Management Association and a Primary Care Trust in England. The
participating organizations engaged in a cross-national exchange and
comparison of transferable good practices in this area and identified
concrete examples of the contribution that the health sector is making to
social inclusion. It especially includes examples from the Czech Republic
and Latvia. 142 pp.

Child Morbidity and Treatment Patterns

This third comparative report on childhood morbidity and treatment
patterns focuses on the symptoms associated with three of the leading
causes of death in early childhood (diarrhea, cough accompanied by short,
rapid breathing, and fever) and treatment responses to these symptoms. The
analysis is based on data collected on children under age three in 52
developing countries. The data presented in this report show wide
variation both within and between world regions in the prevalence of
diarrhea, fever, and the symptoms of acute lower respiratory infection
(ALRI) in young children. There are also substantial differentials in
maternal careseeking and other treatment responses to early childhood
illness. The survey data include Armenia and the countries of the Central
Asia region. 149 pp.

the Federal Service for Consumer Rights and Human Well-being
(Rospotrebnadzor) said on the eve of the World No Tobacco Day.

‘The number of Russian smokers is among the highest in the world.
Forty-two percent of early death cases among men aged 35-69 are caused by
smoking,’ the press release of Rospotrebnadzor reads.

Every year over 260,000 deaths have smoking-related reasons in Russia.

Recently, the number of smokers has been growing among Russian young
people, and regular smokers are becoming younger. For example, 50% of boys

global event established to call attention to the impact of tobacco use on
public health and reduce individual tobacco-dependence.

The role of health professionals on tobacco control

Health Professionals are in an excellent position that allows them to have
a prominent role on tobacco control.  Comprehensive tobacco programs aimed
at controlling the use of tobacco efficiently should consider a mix of
measures including legislation and pricing measures, but also prevention
-through education, communication, informational campaigns that raise
awareness of the effects of tobacco on health- and other demand reduction
measures concerning tobacco dependence and cessation. Health Professionals
can intervene in these ambits. They reach a high percentage of the
population. Health Professionals have the opportunity to help people
change their behavior and they can give advice, guidance and answers to
questions related to the consequences of tobacco use, they can help
patients to stop smoking. Studies have shown that even brief counseling by
Health Professionals on the dangers of smoking and the importance of
quitting is one of the most cost-effective methods of reducing smoking. 
Health Professionals can also be very instrumental with preventive
measures, especially when considering the youth. They have the opportunity
to promote social norm change, and forewarn children and adolescents of
the dangers of tobacco.

Effectiveness of School Programs in Prevention of Smoking

A literature review. Smoking is the most important preventable cause of
premature death and loss of health in developed countries and effective
smoking prevention represents a very important public health problem.
Since smoking initiation starts as early as in childhood, measures of
primary prevention should be focused on this age group. One of the
possibilities is school-based programs implemented in numerous countries.

Smoking cessation facilitating drugs – a cost-benefit analysis of the
over-the-counter and prescribed drugs

The main objective of this literature review is to present the available

This publication presents an overview of the practices and the context of
pharmacotherapy of opioid dependence in selected countries in Central and
Eastern Europe and the Newly Independent States. Based on reports provided
by professionals involved in the treatment of opioid dependence in these
regions, this document describes the current situation with opioid use in
Central and Eastern Europe and the Newly Independent States, the role of
pharmacological treatment of opioid dependence in public health responses
to opioid dependence and associated health consequences in the region, as
well as priorities and recommendations for development of treatment

Substitution maintenance therapy in the management of opioid dependence
and HIV/AIDS prevention

The World Health Organization (WHO), the United Nations Office on Drugs
and Crime (UNODC) and the Joint United Nations Programme on HIV/AIDS
(UNAIDS), have developed a joint position on substitution maintenance
therapy for opioid dependence. Based on a review of scientific evidence
and oriented towards policymakers, the WHO/UNODC/UNAIDS position paper
covers a wide range of issues, from the rationale for this treatment
modality, to the specific considerations regarding its provision for

Drug Policy and the HIV Pandemic in Russia and Ukraine

Over the past three years Russia and Ukraine have experienced one of the
fastest growing HIV pandemics in Europe. In contrast to other parts of the
world, the main driver behind the rate of infection is injecting drug use.
Recent government policies have placed a heavy emphasis on reducing
availability and on harsh punishments for drug users. This approach has
not succeeded in significantly reducing the level of drug use. It has
pushed the drug scene underground and increased risky behaviors among
vulnerable groups. In the absence of measures to reduce infections and
reverse the rate of transmission, the long-term impact of HIV/AIDS on
population growth and economic development is likely to be grave. 4 pp.

Drugs, their effects and Harm Reduction

The article presents various types of drugs: depressants, stimulators, and
hallucinogens. It also specifies the effects related to use of alcohol,
barbiturates, stimulators, amphetamines, ecstasy, hallucinogens, LSD, and
psylocybe mushrooms. The article defines the term "addiction" and

Injecting Drug Users, HIV/AIDS Treatment and Primary Care in Central and
Eastern Europe and the Former Soviet Union

The report includes the latest statistics on HIV/AIDS cases and treatment,
basic health care for drug users and others with HIV/AIDS, and
substitution treatment such as methadone maintenance. The publication,
prepared on the basis of a 27-country survey of Central and Eastern Europe
and the former Soviet Union, has found that fewer than one percent of an
estimated one million people infected with HIV/AIDS receive the three-drug

on national and local levels, and specifies actions to arrest the epidemic

The aim is to promote a healthy, drug-free lifestyle among vulnerable
youth. PSI/Central Asia's role in this ambitious intervention is to add
drug misuse prevention activities to its core, USAID-funded
HIV/sexually-transmitted infection (STI) prevention program launched in
2002 targeting youth, injecting drug users (IDUs) and sex workers in
Kazakhstan, Uzbekistan, Tajikistan and Kyrgyzstan. Program components will
include mass media, events, educational materials, peer education, youth
service centers, research and community mobilization.

The IHRD program supports local, national, and regional initiatives in

address drug problems through innovative measures based on the philosophy
of harm reduction. IHRD’s mission is to diminish the individual and
social harms associated with drug use—particularly the risk of HIV
infection—through innovative measures based on the philosophy of harm
reduction. This approach places an emphasis on human rights, common sense,
and public health. In conjunction with the Public Health Programs and
Soros foundations, IHRD currently supports more than 200 projects in 23
countries in Eastern Europe and the former Soviet Union.

ZdravPlus

Abt Associates has worked in Central Asia since 1995 under three consecutive projects – ZdravReform (1995-2000), ZdravPlus (2000-2005), and ZdravPlus II (2005-2009).  These regional projects, funded by USAID, have helped the governments of Central Asia improve the financial sustainability, efficiency, and quality of their health-care systems while preserving equitable access.  The current project continues to provide technical assistance to increase the efficiency of resource use, improve health-care quality, redefine patient rights and responsibilities, and create a favorable legal and policy framework for health reform.  In Kyrgyzstan, the introduction of a new health financing system created system-wide efficiencies, increasing funding for outpatient care and medicines and reducing the excessive number of buildings and staff.  The new system increased cross-subsidies and risk protection for poorer populations, resulting in better access to care for the poorest populations, and substantially reducing the negative impact of key illnesses.

Descriptive Background to Health Care Financing Reform Strategy Development in Georgia

The purpose of the paper is to contribute to the current thinking within MoLHSA and the Government of Georgia (GoG) about health sector financing reforms. The paper takes a historical perspective in order to set the stage for a common understanding of the issues inherited by the health financing system of Georgia from the pre-“rose revolution” period. It aims to bring these issues into the policy discussion process, which is currently being set by the MoLHSA, and which involves all stakeholders (national experts and policy makers as well as international development partners). Organization and Financing of Primary Health Care in Albania: Problems, Issues, and Alternative Approaches

The USAID-funded PHRplus Project is developing model PHC clinics in four sites in one region of Albania in an effort to demonstrate ways to improve systems performance. One element of the project is to assist the Government of Albania to design and implement improved methods for the planning, budgeting, and financing of these PHC services. A major part of the technical assistance in these areas has been to analyze the complex and disparate sets of data on flows of funding, how they have changed, and how those changes have affected accountability for PHC systems performance. Using these data, this report develops the basis for designing alternative ways to organize and manage the PHC service delivery system, taking account of a recent change in government policy that would unify all funding for PHC in one agency—the Health Insurance Institute (HII). After developing and applying criteria for choosing among two options for reorganizing management of PHC, assuming the HII would become the single source of financing, this paper recommends an alternative that would create regional health offices to supervise and manage PHC in each region, with increased levels of autonomy for individual PHC practices. The report also describes the principal elements of a proposal for a pilot project to test in one region the implementation of the recommended reorganization of PHC using the HII as a single source of PHC financing. 92 pp.

Primary Health Care Reform in Albania: A Pilot Project to Provide Evidence for Health Policy

The U.S. Agency for International Development-funded Partners for Health Reformplus Project provided technical assistance to the government of Albania from 2001 to 2005 in the design and implementation of a sub-set of its health sector reform strategy, focused on strengthening the primary health care (PHC) sector. An integrated PHC service delivery model was designed, implemented and evaluated in two districts to inform national health policy, for later refinement and replication on a wider scale. The model integrated five major components - family medicine training for PHC providers, a facility-based health information system, service delivery reorganization and quality improvement, financing reforms, and community involvement. The model addressed the low quality of primary health care, lack of any data on PHC patients or costs, bypassing of PHC clinics for specialty polyclinics or hospitals, fragmented financing and management of PHC, and the absence of community involvement. The pilot was a bottom-up approach that informed the development of an integrated PHC delivery system that is affordable to the Albanian economy. 78 pp.

The Pilot Process: Case Study on Piloting Complex Health Reforms in Kyrgyzstan

The concept of piloting has been effectively used in implementing health sector reform throughout the former Soviet Union. One of the first pilots in Central Asia, established in 1994 in Issyk-Kul oblast (province or state) in Kyrgyzstan, continues to provide valuable information on the process of piloting complex health reforms. The primary objective of the pilot in Issyk-Kul oblast was to develop or refine a health system design, specifically to demonstrate the feasibility of a mandatory health insurance scheme. Other objectives included demonstrating specific reform designs to provide information and evidence to national stakeholders over time and to simultaneously build capacity for further implementation. This case study describes the pilot process in Kyrgyzstan, including factors involved in the development of the pilot approach, as well as dimensions and outcomes of the pilot. 49 pp.

An Overview of GEOVAC: A Software Application to Monitor Immunization Performance in Georgia

The GEOVAC software application is a tool designed to help personnel of regional level centers of public health and the National Center for Disease Control in Georgia process a large flow of immunization-related data in much less time than the previous (manual) system. It allows them to quickly identify issues and deficiencies regarding immunization coverage, and use and distribution of vaccines, and to assess adequacy of supplies as well as major barriers (medical contraindications, parental refusals, etc.) to the functioning of the immunization system. In doing so, GEOVAC gives health workers more time to focus on the utilization of MIS data for management and disease outbreak response purposes. This second version of the application has gone through numerous revisions and suggestions based on testing in the pilot region. It is now being used nationwide. This document illustrates GEOVAC functions, relating them to the features of the upgraded Georgian immunization information system and demonstrating what it can offer immunization managers in the decision making process. It is designed primarily for policymakers in countries planning to strengthen their immunization and/or surveillance systems, donor organizations that can support such reforms and agencies working in these technical areas. It can also help policymakers and health workers in Georgia to plan and implement similar reforms in other sectors of the health care system. 35 pp.

Assessment of Private Sector Prospects for Reproductive Health and Family Planning Products and Services in Ukraine

The USAID/Ukraine mission requested technical assistance from the private sector program core task order in assessing the ability of the private sector to ensure sustained contraceptive supply in Ukraine. In particular, the PSP-One team was expected to address the current and estimated future availability, affordability, and accessibility of contraceptives in the private sector. This report presents the team’s findings, provides recommendations for activities and policies that can increase private sector engagement in the contraceptive field, identifies specific population groups not likely to be adequately served by the private sector, and suggests strategies to meet their needs while avoiding negative impact on the growing contraceptive market. 33 pp.

Protecting the Human Rights of Injecting Drug Users: The Impact of HIV and AIDS

This International Harm Reduction Development Program (IHRD)-produced volume argues that unduly strict interpretation of UN drug control treaties undermines HIV prevention efforts by discouraging countries from implementing effective, realistic, and compassionate public health policies. The publication's contributors—including Open Society Institute president—demonstrate that international bodies and national governments must give greater consideration to the human rights - particularly access to HIV treatment - of injection drug users. Child Morbidity and Treatment Patterns

This third comparative report on childhood morbidity and treatment patterns focuses on the symptoms associated with three of the leading causes of death in early childhood (diarrhea, cough accompanied by short, rapid breathing, and fever) and treatment responses to these symptoms. The analysis is based on data collected on children under age three in 52 developing countries. The data presented in this report show wide variation both within and between world regions in the prevalence of diarrhea, fever, and the symptoms of acute lower respiratory infection (ALRI) in young children. There are also substantial differentials in maternal careseeking and other treatment responses to early childhood illness. The survey data include Armenia and the countries of the Central Asia region. 149 pp.

The role of health professionals on tobacco control

Health Professionals are in an excellent position that allows them to have a prominent role on tobacco control.  Comprehensive tobacco programs aimed at controlling the use of tobacco efficiently should consider a mix of measures including legislation and pricing measures, but also prevention -through education, communication, informational campaigns that raise awareness of the effects of tobacco on health- and other demand reduction measures concerning tobacco dependence and cessation. Health Professionals can intervene in these ambits. They reach a high percentage of the population. Health Professionals have the opportunity to help people change their behavior and they can give advice, guidance and answers to questions related to the consequences of tobacco use, they can help patients to stop smoking. Studies have shown that even brief counseling by Health Professionals on the dangers of smoking and the importance of quitting is one of the most cost-effective methods of reducing smoking.  Health Professionals can also be very instrumental with preventive measures, especially when considering the youth. They have the opportunity to promote social norm change, and forewarn children and adolescents of the dangers of tobacco.

Drug Policy and the HIV Pandemic in Russia and Ukraine

Over the past three years Russia and Ukraine have experienced one of the fastest growing HIV pandemics in Europe. In contrast to other parts of the world, the main driver behind the rate of infection is injecting drug use. Recent government policies have placed a heavy emphasis on reducing availability and on harsh punishments for drug users. This approach has not succeeded in significantly reducing the level of drug use. It has pushed the drug scene underground and increased risky behaviors among vulnerable groups. The aim is to promote a healthy, drug-free lifestyle among vulnerable youth. PSI/Central Asia's role in this ambitious intervention is to add drug misuse prevention activities to its core, USAID-funded HIV/sexually-transmitted infection (STI) prevention program launched in 2002 targeting youth, injecting drug users (IDUs) and sex workers in Kazakhstan, Uzbekistan, Tajikistan and Kyrgyzstan. Program components will include mass media, events, educational materials, peer education, youth service centers, research and community mobilization.

This approach places an emphasis on human rights, common sense, and public health.
